The gay man, who had been taking the drug daily since January, encountered a drug-resistant strain of the illness.
Another man who was taking daily PrEP medication has become infected with HIV.
According to BuzzFeed News, a gay man who had been taking daily doses of the HIV-prevention medication contracted the virus after a sexual encounter with a partner who had a drug-resistant strain of the disease.
The first case of an infection while on the drug, which has been hailed a sexual-health revolution for gay men, was announced in February.
While there is concern about the drug's effectiveness, health experts say that PrEP has never been 100 percent effective and that a small amount of infections are reasonable.
Despite having a significant risk for exposure to HIV, gay and bisexual men still have little knowledge of what PrEP is and how to use it effectively when engaging in same-sex relationships.
